The U.S. Postal Service's Form 1583


For Users of Remote Addresses Operated by Earth Class Mail

Users of Earth Class Mail Remote Addresses must complete the Form 1583 provided by the U.S. Postal Service. (Enterprises who have mail sent to their own addresses do not need to complete one.) While Document Command's concept of putting all your mail online is totally new, the idea of a person like you having your own mail agent is not. That's why the U.S. Postal Service provides its Form 1583 for you to designate us as your official agent for receiving mail.

For every Earth Class Mail Remote Address you wish to use, we must have an original and a copy of the Form 1583 on file. With exceptions applicable to only a few Earth Class Mail Remote Addresses, residents of the United States must include their notarized signature on the original (the Postal Service does not let us accept faxed, copied, or e-mailed forms).
